Diabetes Wellness Program
Welcome to the Health Choice Diabetes Wellness Program. This program was developed at the request of Methodist Le Bonheur Healthcare in order to offer healthy lifestyle incentives and rewards to Methodist Associates, spouses and dependents age 16 or over who have diabetes.
Eligibility Requirements - You must be enrolled in the Methodist health plan.
- You must be an Associate, spouse, or dependent age 16 and over who has been diagnosed with diabetes.
- You must complete the enrollment form and Health Status Survey. The survey, which gives us information about your diabetes history and general health, will take most people about 20 minutes to complete.
You may enroll in the program at any time during the year. Eligiblity for program benefits begins on the first day of the month following enrollment in the program. For example: If you complete the enrollment form and health status survey on January 8, 2012, you will be eligible for program benefits on February 1, 2012.
Please note: Your personal health information will not be shared with anyone at Methodist Le Bonheur Healthcare. However, if you wish to participate in the insurance premium reduction offered by the Healthy Rewards Program, we will have to provide verification of your participation and successful completion of program requirements to Methodist Le Bonheur representatives.
