Health Choice represents the network of physicians and facilities that your employer has selected to serve as your health care providers.  We can assist you in determining which facilities are considered "in-network" for your plan. 

Start by specifying any of the following: facility's name, type of service you're looking for, and the network/plan.  Then click the "Search" button.
