Registered Clinical Dietician

Position Description:  The Health Choice Registered Clinical Dietician will serve as an integral member of the care team and is responsible for formulating and implementing optimal nutrition therapy and evaluating outcomes for assigned patients. Coordinates nutritional care and educational programs with other members of the HealthChoice team.

Job Responsibilites:

  • Provides nutrition assessment to patients identified requiring medical nutrition therapy. 
  • Formulates medical nutrition therapy plan of care. 
  • Evaluates, interprets, monitors and document patient’s responses to the medical nutrition therapy plan of care and adjusts intervention to maximize therapy as indicated by the patient’s condition. 
  • Educates patients, significant others, care-providers and identified staff regarding medical nutrition therapy, lifestyle changes, and nutrition needs. 
  • Documents the care delivered and communicates essential information to the patient and their primary care providers. 
  • Provides information, support services, and resources designed to enhance patient adherence with his or her medical nutrition therapy plan of care. 
  • Provides specialized services to help patients manage conditions such as diabetes, asthma, smoking cessation, or high blood pressure. 
  • Refers patients to other health professionals and agencies when appropriate. 
  • Coordinates and integrate medical nutrition therapy within the broader healthcare-management services provided to the patient. 
  • Participates in quality improvement initiatives related to the medical nutritional therapy program and educational initiatives. 

Education/Training Required:

  • Bachelor’s Degree in Nutrition or Clinical Dietetics. 
  • Registration with the Academy of Nutrition and Dietetics (AND) or registry eligible graduate who obtains registration within one year of employment. (See exceptions allowed). 
  • State license in accordance with regulations in the state where work is performed, if state license is required. (See exceptions allowed). 
  • Inter-State transfer associates, who have current registration, must obtain state licensure (if state license is required) within eight months of employment. 

Exceptions allowed:

  • New graduates that are registry eligible must obtain registration with AND within one year of employment. 
  • State license (if license is required in the working state) must be obtained within eight months of Registration with AND within one year of employment. 

Preferred:

Certified Diabetes Educator through the National Certification Board of Diabetes Educators, highly desired.
