Open Position: Certified Medical Assistant

The Certified Medical Assistant is responsible for assisting and supporting the Care Management Team through telephonic outreach to assist members with finding the right place for care, gathering clinical information to optimize care coordination, and facilitate closing gaps in care. 

The successful candidate will be an enthusiastic and flexible self-starter who can work to facilitate sharing clinical information across the continuum of care, educate and empower our members to select the right place for care, and assist the care managers in the care coordination.  Practice in the clinical or family practice office setting is preferred. Excellent written and verbal communications skills are a must.

Certified Medical Assistant Job Purpose:  Assists Nurse Case managers in the transition of care process by helping to secure needed patient records, outreach to patients to set up follow up appointments, assess patient needs for additional resource needs. Works with medical practices to make sure they have the needed information to optimize care of their patients including information on gaps in care, specialist treatment, ED utilization etc.

Certified Medical Assistant Job Duties:

  • Secures patient records from individual providers or healthcare facilities.
  • Facilitates the sharing of medical information across the continuum of care.
  • Updates practice EMR records with pertinent information.
  • Maintains and completes records by recording patient examination, treatment, and test results.
  • Assists patients in making follow up appointments.
  • Educates patients on appropriate use of the emergency room, primary care, specialists and urgent care.
  • Provides patients necessary educational materials about medical condition.
  • Refers patients to available/appropriate resources.
  • Verifies patient information by interviewing patient; reviewing and/or recording medical history.
  • Establishes sound working relationships with medical practice teams.
  • Maintains safe, secure, and healthy work environment by following, and enforcing standards and procedures; complying with legal regulations.
  • Maintains patient confidence and protects operations by keeping patient care information confidential.
  • Adheres to professional standards, policies and procedures, federal, state, and local requirements, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards.
  • Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

Education/Training Required:

  • Graduate of a technical school medical assistant program.
  • Preferred: Current certification for Registered Medical Assistant (RMA) through the American Medical Technologists (AMT) or the certified assistant through the American Association of Medical Assistants (AAMA).

Relevant Experience Required:

  • 2 years of related experience; or any combination of education and experience, which would provide an equivalent background. Experience in a family practice setting would be most advantageous.
  • Previous experience with an electronic medical records system highly preferred.
  • Must be knowledgeable on medications and have a strong medical vocabulary.
  • Strong written and verbal communication skills with the ability to work with patients, patient families, and physicians in a tactful and professional manner.
