Chronic Kidney Disease Program

Dealing with the implications of Chronic Kidney Disease can be stressful and scary.  The HealthChoice Chronic Kidney Disease (CKD) Program provides education about this condition and an opportunity to ask questions and share your experiences with others.  Our clinical staff will lead you through the causes of CKD, teach you ways to prevent or delay its progression and help you make educated decisions about your treatment options.

This yearlong interactive group program is comprised of eight weekly one hour sessions, followed by six monthly one hour sessions.  Participants will have a chance to learn new information, ask questions and talk with experts about their condition.

The CKD program is free of charge.  In the program, you will learn:

  • How your kidneys function
  • How to prevent complications
  • How to eat right for kidney health
  • Important facts on medications, targeting the drugs used to manage kidney disease as well as high blood pressure and diabetes
  • Tests used in monitoring kidney disease
  • Treatment options for advanced kidney disease and how to make educated decisions
