Chronic Kidney Disease and Dialysis Transition
Learning that you have kidney disease and may need dialysis can cause fear and uncertainty for the future. Maintaining a healthy and productive lifestyle is important. We provide care coordination based on specific needs related to Chronic Kidney Disease (CKD), End State Renal Disease (ESRD) and Dialysis. Our goal is to slow the progression of CKD and delay the need for dialysis if possible. We can help you TAKE an ACTIVE ROLE in your care and live well with end stage renal disease.

Complex Care Management
Through our Care Transition Support program, we identify members who may have a medically complex illness or multiple chronic conditions. Juggling doctor visits, hospital and ER visits, or multiple medications, can be very overwhelming. Our Complex Care Manager can help you take control and navigate confusing healthcare systems. We provide education and care coordination based on specific needs with the goal of enabling you to manage your care independently.

HealthChoice as an ACO
HealthChoice is also an Accountable Care Organization (ACO). Our network of doctors, hospitals, dialysis centers and other healthcare providers work together with our Nurse Care Managers and Medical Assistants to coordinate your medical care.
