Dealing with the implications of Chronic Kidney Disease (CKD) can be stressful and scary.  Being told you have kidney failure and require dialysis can be overwhelming.  This program ensures you receive education about treatment options and the opportunity to ask questions and share your experiences.  Our care management staff will help you find the resources you need to learn about your condition.  This includes when to seek treatment, how to prevent complications, and what can be done to help delay disease progression.  You should know the treatment options that work best for your situation.

If your Nephrologist is speaking with you about dialysis, our Nurse Care Managers can help you transition to the right facility to meet your needs.  Your health plan benefits may require you to seek care within a specific network of dialysis centers.  We can help you navigate that process to ensure your do not incur any unnecessary expense.
